Discharge

Discharge plans for child and adolescent patients are practical and personalized, ensuring a smooth transition from our care and with consideration for the unique needs of each patient and family. We account for clinical indications, such as the patient's progress and ongoing therapeutic needs, as well as family preferences and the financial or insurance realities that may impact their options.

“Everyone was so amazing. I called and got an appointment the same day. everyone was so welcoming and kind. And I was reluctant to go , but the doctor Elizabeth made me feel so comfortable and was very attentive. I strongly recommend this office. I was calling around for 2 days trying to get an appointment at different places, they were all booked for months out. Then. I came across Halo, and was shocked when they said I could come in same day , and  I'm really glad I did.”

"Provided me with the direction, support, and adjustments to my medications that I desperately needed. I was in a position where I felt that I was not in need of hospitalization, but needed treatment that same day. I called and made a same day appointment. I was able to discuss and adjust my meds and have a therapy session that same day. I was provided arrangements for continuing care with long term providers and was able to continue treatment at Halo until those appointments were established.   Highly recommended. Thank you very much for this service. Thank you to the entire Halo Team. :)"
